ஒரே ஒரு புடவயால்தன் மானம் காக்க மார்பகத்தையும்மற்றவற்றையும்லாவகமாகஅணிந்த்திருக்கும் நம் கிராம நங்கை பயத்தையும்நடுக்கத்தியும்அடக்கிக்கொண்டு கேட்கிறாள் “ஐயா மருத்துவர் எங்கே பார்க்கலாம்?”

Reaches the reception counter. Wonders she is directed to stand behind a very tall Sardar in a line. Her worry was how she will be spotted since the giant in front seems to hide everything behind him. To her relief the ‘giant’ was told that he was in wrong queue and asked to move away. Girl behind the reception counter, rouged, painted eye lashes,and sporting lipstick so loud that you can almost hear it. “Yes Madam” she addresses our நங்கை.
நம் நங்கையின் ஐயம் “இந்த பொண்ணு சினிமா ஸ்டார் மாதிரி இருக்கா “ எனினும் “தாயீ தமிழ் தெரியுமா?” சினிமா ஸ்டார் “என்ன வியாதி உனக்கு? (not உங்களுக்கு). பாட்டி” “ஐயே நான் பாட்டி ஒன்னும் இல்லை சின்ன வயசு தான் “ ஸ்டார் “ என்ன கஷ்டம் சொல்லு” பாட்டி “ உடலெல்லாம் வலி” ஸ்டார் “எங்கே சொல்லு , தலையிலா ,வயிற்றிலா சொல்லு” பாட்டி “உடம்பு பூரா” ஸ்டார் தன் தோழியிடம் “This woman cannot specify her complaint. நமது நங்கை—பாட்டி. “ டாக்டர் சாமி கும்படேறேன் “டாக்டர் “என்ன மாரி வேடிகாலேலேவந்துட்டே. வீட்டுக்காரன் பணம் கொடுக்கிறானா?” பாட்டி “குடிச்சது போக மிச்சம். ரெண்டு நாளாலேசா ஜுரம் உடம்பு பூரா வலி. குணப்படுத்துங்க. பிள்ளைகளுக்கு சரியா பொங்கி போடணும் “ டாக்டர் “இந்த மாத்திரை மருந்து ரெண்டு நாள் சாப்பிடு. உன் வீட்டுகரனை என் கிட்டே அனுப்பு. அவனுக்கு சொல்லவேண்டியதை சொல்லிவிட்டு அவனையே காசு கொடுக்க சொல்றேன்.”
இரண்டுமே என் கற்பனை அல்ல. நான் நேரில் கண்ட கேட்ட உண்மை.
Polar extremes of “health care delivery systems” existing in our country is highlighted by these true stories. My attempt is to focus on “நம் வீட்டு வைத்தியன்” =Primary physician=GP.Rural Indian as well as considerable percent of metropolitans still trusts their GP. But GP is ridiculed & derided by “Super Specialists” (like Nuevo Rich in general population). When Gastroenterology which is a tiny part of General Medicine, is labelled “SUPER specialty” it is unjust. It is indeed a “SUB specialty” (Army Medical Corps used the term “Sub specialty”. An expert in any Subspecialty knows more & More about less & less. He wears blinkers and is condemned to a tubular vision. Comments like “ Oh these GPs, cannot treat properly, cannot refer in time “ are as common as they are frustratingly disgusting. How much better if the specialist says “ GPs have their limitations, no infrastructure support, no PG to feed him with latest, Still he has done his best and referred at right time”. Such benevolent remarks are rare if ever. Somehow praising a fellow clinician is not common among physicians. Why is it the specialist never endorses a note to the discharge summary explaining to the patients GP problem encountered, how resolved, what should GP focus during follow up and when, if at all, patient may be sent for review. This will have two favorable outcomes. Patient is thankful for the transparency. And GP feels gratified and encouraged.
A note from “Obama care”: ": I think that having a doctor -- your doctor -- matters; having someone to watch over you makes a difference, helping you get the care you need, helping you through an acute illness, helping you deal with your chronic medical conditions, helping you address your barriers to getting the care you need, helping you make sure you don't make the missteps that lead to irreversible problems or bad outcomes. Everyone in America would benefit from having a primary care doctor.
A striking difference between GP &Specalist: GP listens and physically examins.
A quote from BMJ: If an alien anthropologist were to visit a modern teaching hospital, "it" might conclude that, judging by where doctors spend most of their time, the business of an internal medicine service takes place around computer terminals. The alien might assume that the virtual construct of the patient, or the "iPatient", is more important than the flesh and blood human being occupying the bed.
Tail piece: Prof KS Sanjeevi an eminent teacher in my studentdays told us this during a ‘bedside’. (He was soft-spoken but his jokes were subtle & deep. . He started VHS (Voluntary Health Service) in Adyar. “One evening as my wife & I were about to get into the car a person came running in “Aiya are you Dr.Sanjeevi?” “Yes” said the venerable prof. “Saar, please examine me and prescribe”. Prof” As a consultant, I don’t see patients unless referred by another doctor. So, see a doctor & come”. Patient “ Saar,I visited TEN clinics in Luz &Abhiramapuram. Everywhere there was a big crowd of patients. Only in your house there is none.” Prof.Sanjeevi asked him to come in !!
